VOLUNTARY TRANSPORT

Getting too and from a doctor/hospital appointment can sometimes be very difficult if you do not drive or do not have anyone that can take you.  This can cause added stress when you are already feeling under the weather.

Listed below are the contact numbers for organisations that may be able to assist you attend your doctor/hospital appointment:

Continuing Care Volunteer Service – 01334 880743 – (a minimum payment of £5.00 for petrol)

For those who live in an area of Fife where there is no local bus service the Go-Flexi Taxibus may be helpful. This is a pre-bookable service, (1 hour to 1 week in advance) and normal bus fares apply. If interested, please call 01382 540 624.

New Patient Register

To make things easier for you to register, download a Registration Form  and Questionnaire Form.  Complete these at your leisure, print them off and bring them into the practice. It is practice policy to ask for the completed forms to be accompanied by photographic ID and proof of your address. Click here for more information on the Registration process.

The practice has a dedicated Mental Health Triage Nurse working with the practice team  offering patient appointments on a Tuesday each week.
